The Brockton and Area Family Health Team (BAFHT)

Is inviting applications for a

Contract Social Worker/Mental Health Therapist

18.75 hours/week for a period of up to 15-months

The successful candidate will work closely with our group of local family physicians, nurse practitioners, and allied health providers as part of a dynamic interdisciplinary health care team.

Main Responsibilities

  • Provide individual counselling using a range of therapeutic modalities for patients with mild to moderate mental health issues under a brief-therapy model.
  • Conduct intake assessments to ensure that patients are being linked to the most appropriate mental health services, considering both internal and external options.
  • Work closely with other team members to provide a collaborative approach to patient care.
  • Support patients with System Navigation to facilitate access to appropriate health and social services.
  • Participate in the planning and implementation of education and treatment group programs.

Position Qualifications

  • Bachelor of Social Work or Counselling degree from a recognized program
  • Current registration with the OCSWSSW or OACCPP
  • Demonstrated skills in assessment, program planning, implementation and evaluation
  • Recent primary care or community-based counselling/therapy experience
  • Demonstrated ability to work collaboratively within an interdisciplinary team
  • Excellent interpersonal, communication and organizational skills
  • Experience coordinating patient care using community resources
  • Proficiency in the use of computers and information technology
  • Valid driver’s license and access to a reliable vehicle
  • Preferred: Master of Social Work
