Social Worker/Mental Health Counsellor

Position: Social Worker/Mental Health Therapist (October 2024)

FTE Type: Full-time (part-time considered)

The Hanover Family Health Team is seeking a collaborative Social Worker/Mental Health Therapist to join our primary care practice on either a permanent full-time (37.5 hours per week) or part-time basis (up to 22.5 hours per week). This clinician will assist with referrals for mental health services and provide brief/short-term therapeutic counselling services. This role also supports our Memory Clinic program and works alongside our Memory Clinic interdisciplinary team. This role also connects with patients to review broader health and social needs and provide information and referral to support patient and caregiver system navigation.

Requirements

Master in social work, counselling, psychology or equivalent in a clinically relevant field from an accredited university with a minimum 5 years mental health related work experience.

Advanced generalist counselling skills to be used to provide brief /short-term counselling to assist in the treatment of an array of adult mental health and psycho-social problems including, but not limited to: depression, anxiety, adjustment problems, bereavement and loss, stress, work problems, domestic violence, trauma, chronic pain, aging issues, suicidal ideation and crisis planning, eating disorders.

Ability to work independently within scope of practice and also collaborate in an inter-disciplinary team to support patient needs.

Registered with one of the following regulatory colleges: Ontario College of Social Workers and Social Services Workers; College of Occupational Therapists of Ontario; College of Registered Nurses of Ontario; College of Registered Psychotherapists of Ontario. 

Additional clinical certification or experience in a specialty area is an asset, e.g. trauma, children and youth.

Reliable transportation to work.

Up-to-date vaccinations (or willing to get up-to-date).
