Case Manager (Private, Family Hire)

POSITION SUMMARY

The selected candidate will work as a case manager for my two adult siblings to assist with transitional planning and securing work/volunteer opportunities. You must be proactive, possess excellent communication and organizational skills, and be committed to prioritizing their needs and interests in your work. 

The family has already taken some steps to try to begin transitional planning and is in need of the assistance of someone with familiarity and experience in this area to help move things forward. 

Note: As each sibling has their own unique needs, including one of them requiring communication in American Sign Language, there may be more than one position available. 

 

RESPONSIBILITIES

  • Meeting with the family to understand their needs, interests, and goals

  • Developing a case plan accordingly, monitoring and evaluating progress 

  • Transitional planning with a focus on achieving semi-independence, including housing options and securing work or volunteer opportunities  

  • Searching for and contacting relevant services, programs, and opportunities within the Kanata and Ottawa area, according to their individual needs 

  • Sharing these findings with the family and making recommendations on which ones should be pursued

  • Contacting selected services to begin registration processes, arrange interviews, etc. 

  • Provide updates at least once weekly and engage in consistent, open communication with the family 

 

QUALIFICATIONS

  • The selected candidate will possess at least one of the following: Master of social work, Bachelor of social work, Developmental services worker diploma, Social service worker diploma 

  • At least four years of professional experience working with persons with disabilities (personal family experience will also be considered)  

  • Research and advocacy skills

  • Experience in transitional planning, case management, referrals services and/or coordination, or similar are required 

  • Preferred candidates will have conversational skills in American Sign Language (ASL). Qualified applicants who do not have ASL skills may work with our Deaf adult through Video Relay Service (VRS), i.e., remotely

  • Clear Police Reference Check with Vulnerable Sector Screening

 

TERMS OF EMPLOYMENT: Part-Time, exact hours to be determined with the selected candidate. Salary to be discussed with the selected candidate.
