SOCIAL WORKER – PART-TIME (0.6 FTE)

Family Health Teams (FHT’s) use a patient-centered approach to primary health care that brings together physicians and other health-care professionals to co-ordinate the highest possible quality of care for patients – when they need it, as close to home as possible. FHTs are a key component of Ontario’s health-care plan to reduce wait times, improve access to doctors and nurses, and keep Ontarians healthy. 

The Dufferin Area Family Health Team is seeking a Social Worker (3 days, 22.5 hours per week) to join our dynamic, innovative, compassionate team dedicated to supporting, empowering, and collaborating with patients for improved health and well-being. This hybrid position will be in Dufferin-Caledon, including Bolton.

We are looking for someone to join our team who is…

  • Seeking an opportunity to develop professionally in a progressive, collaborative, and highly engaged multi-disciplinary health team
  • Effective and responsive in supporting, advocating for, and collaborating with individuals struggling with complex needs, including disability, poverty, illness, trauma, addiction, palliative, or mental health concerns
  • Knowledgeable of financial programs and able to support individuals navigating an application process for ODSP, Ontario Works, or CPP
  • Skillful in conducting risk assessments and co-developing safety plans
  • Aware of palliative and hospice services/supports in Dufferin-Caledon and the surrounding area
  • Flexible, energetic, organized and committed to working from a trauma-informed, strength-based perspective
  • Respectful of diversity, including race, religion, culture, sexual orientation, and gender identity

Experience needed to be considered…

  • MSW (with 2+ years experience) or BSW (with 4+ years experience) (Required)
  • Membership in good standing with OCSWSSW
  • Demonstrated case management, system navigation and referral coordination experience
  • Current up-to-date local knowledge of Dufferin-Caledon community and health services and PHIPPA
  • Proof of Covid 19 immunization and Criminal Records Check/Vulnerable Sector Screening
  • Valid Ontario Driver’s License, valid insurance, and access to a reliable vehicle
