The Alliston Family Health Team (AFHT) is currently seeking a Social Worker – Masters Level. There is an option for temporary full time or temporary part time to cover a maternity leave with potential for permanent employment within our team.
As a key member of the Alliston Family Health Team (AFHT) and an integral part of an interdisciplinary healthcare team, you will provide mental health counseling / therapy and other therapeutic services for individuals and groups as an interdisciplinary team. Your primary roles are to work in partnership with the AFHT to perform patient assessments, provide short term counseling, education and advocacy.

Responsibilities and Duties

  • Collaborate with the family physicians and nurse practitioner
  • Work independently and effectively as part of a team
  • Provide individual, and group counseling
  • Effectively triage and manage a social work caseload
  • Maintain up to date knowledge of community resources and services and facilitates referrals as appropriate
  • Provide timely written documentation in the electronic health record using professional standards
  • Maintain client, team and organizational confidentiality
  • Utilize appropriate clinical supervision
  • Promote awareness of other services and programs within the Alliston Family Health Team
  • Maintain monthly and quarterly data collection
  • Participate in ongoing program evaluation
  • Participate in staff, team and committee meetings ( Mental Health Team ) or task groups for program planning, development and evaluation
  • Demonstrate excellent interpersonal, verbal and written communication and organizational skills
  • Maintain professional standards and registration with the Ontario College of Social Workers and Social Service Workers ( OCSWSSW )
  • Maintain and manage referral database
  • Qualifications and Skills
  • Masters of Social Work
  • 5 years recent experience in clinical social work in a community setting
  • Group work experience
  • Knowledge of applicable legislation acts
  • Member of the OCSWSSW
  • Ability to work independently and as a part of an interdisciplinary team
  • Ability to work a flexible schedule including some evening hours
  • Ability to use electronic medical record, experience with PSS Telus is preferred
  • Excellent Excel, Word, Powerpoint skills

Required education:

  • Masters of Social Work

Required experience:

  • Social Work: 5 years

Required license or certification:

  • Member of OCSWSSW

Job Types: temporary full time or temporary part time to cover a maternity leave with potential for permanent employment within our team.
 

Part-time hours: 37.5 per week

Schedule:

  • 8 hour shift
  • Day shift
  • No weekends

Education:

  • Master's Degree (required)
