The Minto-Mapleton Family Health Team (MMFHT) offers the opportunity to join an innovative, community-based primary health care team serving Drayton, Palmerston, Harriston and Clifford areas. Due to a retirement, the MMFHT is hiring a permanent, full-time Mental Health Therapist (MHT) for our Drayton office which is a 35 minute drive from the Region of Waterloo and the City of Guelph. The MHT, as a member of a multi-disciplinary primary health care team, provides therapy to patients with mild to moderate mental health issues through individual and group counselling and is a resource to family physicians, and other allied health professionals and the community. The MHT will provide counselling/therapy/treatment plans designed to help the individual to perform daily tasks, self-management coaching and support to develop the skills to live independent and productive lives.
Requirements for the position include:
- Master of Social Work preferred and registration with the College of Social Workers and Social Service Workers
- Ability to work independently and as part of a collaborative team
- Prior experience in assessment and counselling in a primary care setting is considered an asset
- Program development, facilitation and evaluation skills
- Strong leadership, interpersonal, problem-solving and organizational skills
- Ability to pioritize, manage time effectively and be flexible within a dynamic work environment
- Excellent oral and written communication skills
- Access to method of reliable transportation
- Knowledge and proficiency in the use of computer and communication systems including email and electronic medical records (EMR)
- Up to date immunization including COVID vaccine
