The Social Worker works within a continuous quality improvement framework, which reflects VHA’s vision and mission of services as well as promoting and maintaining good public relations with clients, customers and VHA staff, volunteers and associates.

Responsibilities:

  • Provide Consultation, recommendations and interventions to clients in their home using evidence based best practice.
  • Assess psychosocial needs of clients and their families in response to illness/changing care needs and their related ability to adjust and/or cope.
  • Counsel clients and their family members concerning identified problems.
  • Counsel clients and their family members concerning long range planning.
  • Provide short-term therapy to clients and family members.
  • Problem solve in collaboration with client and care team to create opportunities for more independence.

 

Education and Experience:

  • Master of Social Work Degree from a recognized university.
  • In good standing with the Ontario College of Social workers and Social Service Workers.
  • Minimum 2 years of Clinical experience.
  • Valid driver's license with access to a vehicle.
  • Bilingual in French and English
