Position: Social Worker
Status: Permanent Full Time (2 Positions available)
Hours of work: 36.25 hrs/weekly
2021 Salary: $56,012 - $71,403 annually
Location: Brockville, ON

The position of the Social Worker is responsible for providing a broad range of clinical and support casework services to developmentally disabled clients (adults and children) and other individuals (parents, guardians, primary caregivers, etc.). Responsible for gathering information, coordinating the development and implementation of Individual Support Plans, they will liaise with inter- and intra-agency personnel and advocate on behalf of clients and families.
QUALIFICATIONS AND REQUIREMENTS:
- Master’s degree in Social Work (registered in Ontario)
- Member in good standing - College of Social Work
- 5 years related experience
- Knowledge needs and best practice service standards for persons with developmental disabilities
- Excellent mediation and conflict resolution skills
- Well-developed support and interview skills to assist client facilitation
- Able to work with multi-disciplinary team members
- Good interpersonal skills including the ability to build rapport and trust
- Excellent oral and written communication skills
- Able to explain abstract concepts to clients and professionals of varied backgrounds and abilities
- Sound judgment and interpretation skills
- Sound analytical skills with extensive experience with evaluation and test analysis documentation
- Very good relationship management, conflict resolution and mediation skills
- Understand a variety of learning styles/communication techniques to best impart knowledge
- CPI, CPR and First Aid
- Valid Ontario driver’s license and access to a reliable vehicle
DUTIES AND RESPONSIBILITES:
Provide services to clients:
- Provide direct, individual, family and group casework- counseling and therapy.
- Participate in the gathering of information for the purpose of identifying/clarifying referral questions and formulating recommendations concerning methods of intervention.
- Coordinate/gather information and participate in the development and implementation of service and support case plans; short- and long-term goals, parent/mediator training plans, and Individual Support Plan (if they are Coordinator) as required.
- Participate in the assessment process that may involve measuring of psychosocial, behavioural and adaptive functioning of family dynamics.
- Establish goals and monitor DSLG Contract Support Workers to ensure the support is goal-focused as set out in the ISP, and is meeting the financial targets.
Provide support to clients:
- Develop necessary Individual Support Plans for clients, or the case plans for a specific service.
- Provide support to developmentally disabled individuals and/or their primary care givers by accessing, coordinating and liaising inter- and intra-agency services.
- Advocate on behalf of clients/families.
- Interpret agency services and intervention strategies.
- Provide follow-up services to clients/families during cessation of services.
- Develop a variety of groups to meet the individualized client needs and interests.
Provide administrative support:
- Attend and participate in regular team meetings.
- Actively participate in supervision process, performance evaluation and identification of training needs.
- Maintain records of complicated/specialized cases of clients’ contracts, progress reports, case plans etc.
- Complete case plans for new referrals of eligible individuals.
- Extend services to complex cases and development plans for adults and children.
- Providing support services with respect to anger management and abuse, awareness and prevention and rights and responsibilities.
Related to policies and procedures:
- Operate in accordance with applicable Legislative, Ministry and professional association requirements.
- Knowledgeable about and guidance of others in order to fully comply with Quality Assurance Measures (QAM) guidelines as directed by MCCSS.
- Knowledgeable about, and ensures others are knowledgeable about in order to fully comply with the agency’s policies and procedures.
- Health and safety:
- Adhere to Health and Safety Program requirements as per the Occupational Health and Safety Act.
- Ensure an awareness of safety measures for the agency, yourself and the individuals receiving service.
- Ensure compliance with the agency’s mandatory training requirements.
- Operate, at all times, in a safe and secure manner.
