Family Health Teams (FHT’s) use a patient-centered approach to primary health care that brings together physicians and other health-care professionals to co-ordinate the highest possible quality of care for patients – when they need it, as close to home as possible. FHTs are a key component of Ontario’s health-care plan to reduce wait times, improve access to doctors and nurses, and keep Ontarians healthy.
The Dufferin Area Family Health Team currently has an opportunity for a full-time Mental Health Community Case Manager to join our compassionate, dynamic team dedicated to the health and well-being of individuals and families in Dufferin-Caledon.
Are you …
- Enthusiastic about supporting individuals experiencing mental health concerns and significant psychosocial needs? (i.e., disability, financial stress, illness, housing issues, employment challenges, isolation, loneliness, etc.)
- Able to provide flexible, collaborative case management to assess needs, navigate services, and engage individuals?
- Energetic, persistent, and respectful with demonstrated ability to develop effective relationships with others?
- Experienced in partnering with others to identify strengths and needs, develop goals, objectives, and take concrete action steps towards needs?
- Knowledgeable about health and social services in Dufferin-Caledon and able to be a resource for individuals applying for ODSP, Ontario Works, or CPP?
- Skillful in risk assessment and crisis management?
- Able to demonstrate sensitivity to diversity, whether related to race, religion, culture, sexual orientation, age, and/or gender identity?

- Masters in Social Work with 3+ years of case management experience supporting adults with mental health, addiction, disability, financial and/or other complex needs. An applicant with a Bachelor of Social Work, complemented by 5+ years of case management experience, will also be considered.
- Membership in good standing with the Ontario College of Social Workers and Social Service Workers.
- Ability to work independently, take initiative, utilize sound judgment, be flexible, and collaborate well with others.
- Current understanding of health and social services in the Dufferin-Caledon.
- Ontario Driver’s License, valid insurance, and access to a reliable vehicle.
- Satisfactory Vulnerable Sector Screening (Police Check)
