Overview: Ontario Structured Psychotherapy Program

OSP is a stepped-care program, which means that clients start treatment that is the best fit for them with the shortest wait. As clients go through the program, treatment is adjusted based on whether their progress and other additional needs. Most clients start at the first step, which consists of low intensity treatment offerings. Clients are transferred to the second step, which consists of high intensity treatment offerings. Intensity in OSP refers to the amount of clinician contact. Treatment offerings in the OSP program follows a cognitive behavioral therapy (CBT) framework.

  1. Low Intensity Services

The Clinician Assisted Bibliotherapy (CAB) program is our OSP low intensity treatment offering. The clinician is responsible for providing a combination of assessments (intake/triage), treatment, and interim support to clients with mild to moderate anxiety and depression through a structured program. The breakdown of the functions will be determined by operational needs.

  • Assessments are standardized in alignment with the OSP guidelines.
  • CAB is delivered in a series of sessions over approximately 2-3 months for each client. The clinician provides psychotherapy through remote modalities (such as telephone or videoconferencing)  around specific concepts and activities from selected CBT manuals. Client sessions are typically 25-30 minutes long.
  • The clinician also administers structured questionnaires to measure the client’s quality of life, functioning and symptoms of anxiety & depression.
  • Clinicians may also be required to provide short-term, interim support to patients who are awaiting transfer to higher-intensity services (i.e., supportive, brief contact).
  • The clinician liaises and reports any client concerns or safety issues to the client’s primary health care provider and the CMHA Clinical Consultation team. The clinician maintains client records, documenting clients’ progress through the program. The clinician also receives clinical support and supervision through feedback from Clinical Psychologists and will be required to adhere to clinical directives. The incumbent is also required to perform other related duties as assigned.

  2. High Intensity Services

High intensity services are offered through CMHA’s NLO.  NOTE: As the NLO is currently being developed, the specific required functions of the clinician will be determined by the operational needs of the program. The clinician will provide these functions in alignment with the clinical guidelines of the OSP program. The client population includes those with mild to moderate depression, anxiety and anxiety-related conditions.

In general, the clinician is responsible for providing assessments (intake/triage), treatment, and interim support to clients with mild to moderate anxiety and depression,  through a structured program. The breakdown of the functions will be determined by operational needs. There may be some clinicians who are focused more on intake/triage and others who are more focused on treatment. Please note that intake/triage is provided remotely (over the phone or via videoconferencing) and that a portion of high-intensity services will be required to be offered in-person at a Satellite Delivery Site within the CMHA OSP network.

  • Assessments are standardized in alignment with the OSP guidelines.
  • CBT manuals specific to client presenting problems (i.e., depression, trauma, anxiety) are used in high intensity services. Clinicians hold 60 minute weekly therapy sessions with clients, in various modalities (e.g., telephone, virtual, and in-person). CBT protocols vary from 12-20 sessions.
