DEPARTMENT:                       SENIORS’ MENTAL HEALTH – REGIONAL CONSULTATION SERVICE    

POSITION:                              ONE (1) PERMANENT FULL TIME SOCIAL WORKER (0.8 FTE)

SITE:                                       NBRHC – NORTH BAY, COMMUNITY KING STREET CAMPUS

HOURS:                                  DAYS

Pay rate, benefits and other terms and conditions are as per the OPSEU 662 Collective Agreement

 

QUALIFICATIONS INCLUDE:

  • Bachelor of Social Work degree required, Master of Social Work degree preferred
  • Current certification with the Ontario College of Certified Social Workers and Social Service Workers is required
  • Minimum of three (3) years of clinical geriatric mental health experience required, outreach setting preferred
  • Demonstrated knowledge and applied clinical skills in working as an effective interdisciplinary team member supporting older adults with various neurocognitive, mental health, substance use and other neurological conditions required
  • Extensive knowledge of and linkages with relevant community services applicable to meeting the varied needs of older adults required
  • Demonstrated cultural awareness and safety in working with older adults and care partners from diverse backgrounds required
  • Demonstrated clinical competence and inter-collaborative skills to promote, facilitate and integrate best practice principles in geriatric mental health and behavioural health required
  • Knowledge of Social Work treatment interventions and best practices required; additional certifications (e.g. CBT) preferred
  • Certification in the following fundamental core competencies: GPA, P.I.E.C.E.S., DementiAbility Methods, etc. required
  • Proven interpersonal negotiation and problem solving skills to establish and maintain positive working relationships with patients, family care partners, peers and community stakeholders required
  • Excellent verbal and written communication skills to effectively document and convey essential clinical and systems information required
  • Commitment to quality improvement and change initiatives required
  • Proven organizational and time management skills to work effectively in a self-directed manner required
  • Applied knowledge of consent and capacity matters and legislation involving this population (e.g. Public Guardian Trustee, Substitute Decision Maker Act, Health Care Consent Act; Mental Health Act; Long-Term Care Act; PHIPA, etc.) required
  • Proficiencies in navigating relevant technological applications for clinical consultation and professional development (e.g. Microsoft Office, Meditech Expanse, Ontario Telemedicine Network, other virtual platforms [zoom], etc.) required
  • Must have a valid driver’s license, access to a reliable vehicle and accommodate regular travel as required
  • Must demonstrate commitment to ensure personal safety, the safety of fellow workers, patients, volunteers and visitors
  • Must support and contribute to a culture of safety and prevention of adverse health events in this organization
  • Good attendance and work record required
  • Bilingualism (Advanced level English/French) is required. All unilingual candidates are encouraged to apply and may be considered if the identified French requirement has been met or there are no successful bilingual candidates. A formal French test will be conducted to evaluate the proficiency level.
