Social Worker (MSW)- Contract

Position Summary

As a member of the GNPLC healthcare team, the Social Worker provides direct patient care through the provision of psychosocial screening and assessments; you will be responsible for the provision and management of Social Work interventions to individuals and their families in the community. You will assess, intervene, consult and provide counselling for clients and their families in the community by:

 

  • Assisting with crisis management and development of coping strategies
  • Maximizing social, emotional and cognitive functioning
  • Assessing capacity to manage finances and personal care needs
  • Assisting with obtaining alternative housing
  • Assisting with the establishment and maintenance of support networks
  • Assisting with crisis management and development of coping strategies
  • Facilitating chronic disease and mental health related groups
  • Creating clinical reports for internal and external sources
  • Monitor patient goals and outcomes through ongoing assessment and documentation of treatment progress with clear, confidential and concise records on the clinic EMR
  • Perform house calls and community outreach visits / programs as needed
  • Provide information, consultation and education to clients, families, caregivers, staff and community agencies on matters relating to their expertise
  • The Social Worker will also participate in program development and evaluation. 
  • You will work as a liaison and as an advocate to the community  
  • Performs miscellaneous job-related duties as required

Qualifications

  • Master’s Degree in Social Work (MSW)
  • Current registration with the Ontario College of Social Worker and Social Service Workers
  • Thorough knowledge of the Code of Ethics and Standards of Practice documents
  • Demonstrated knowledge of the Mental Health Act and the Substitute Decisions Act
  • Knowledge of social work values and ethics, theory and established practices of clinical social work, and competency in clinical practice, including assessment, treatment and crisis intervention skills
  • Knowledge of community systems and resources for the purpose of networking between the GNPLC and other services
  • Knowledge of and comfort with mental health and addictions diagnoses and treatment approaches
  • Computer skills and EMR experience an asset
  • Both independent and team based client management skills required
