Location: J.E. Stokes Medical Center (Schreiber, ON) and Aguasabon Medical Clinic (Terrace Bay, ON). The Family Health Team provides service at two sites.

Reports to: NSFHT Administrator Closing Date: March 26, 2019 ___________________________________________________________The North Shore Family Health Team (NSFHT) is seeking a permanent full time Social Worker

The NSFHT is a small multi-disciplinary team located in North Western Ontario. We provide primary care to patients in the communities of Terrace Bay and Schreiber. We offer a friendly, collaborative and professional environment at two sites – 15 mins apart.

Position Summary: As an integral member of the North Shore Family Health Team, the Social Worker delivers a wide range of mental health services. These services may include management of mental health issues, education, navigation, case management and advocacy. The social worker will manly service individual adults and groups.

Responsibilities and job expectations:

Provide assessment and treatment for patients referred by their health care practitioner or self referred

• Provide social work intervention as part of an interdisciplinary team and support programs across the Family Health Team, such as Memory Clinic or chronic disease programs

• Assist patients and their families in navigating the health care and/or social service system, which may include case management responsibilities

• Provide groups and/or educational sessions in response to specific needs identified by the Family Health Team

• Participate in health promotion activities such as walking groups or health fairs

• Maintain clear, concise and accurate documentation including use of electronic medical records

• Actively participate in ongoing program evaluation and quality improvement

• Provide education to staff and community about mental health and mental illness

• Participate in committee work within the NSFHT and with partnering agencies as needed

• Advocate on behalf of a client or group of clients as needed

• 37.5 hours/week with the availability to work two evenings a week till 7:00 for clinic and then as needed for groups and/or educational sessions

Qualifications and Skills:

Education and Professional Memberships:

• Master’s of Social Work – BSW with experience will be considered • Current registration with the Ontario College of Social Workers and Social Service Workers

• Must be in good standing with regulatory body

• Valid professional liability insurance (we can guide you in this process if you currently do not have liability insurance)

Skills/Abilities/Knowledge and Experience

• Experience in a community based or clinical health service organization • Excellent clinical skills and experience with assessment, treatment and management of mental health problems commonly seen in primary care

• Experience facilitating group programs

• Case management/system navigation skills and a willingness to research community resources

• Ability to function effectively and efficiently both independently an as a member of the team

• Excellent verbal and written communication skills

• Excellent time-management and organizational skills

• Knowledge of electronic medical records (EMR) an asset
