Program Overview – Full Circle, Community Care
The Full Circle Psychogeriatric team provides community based case management, as well as supportive counselling to older persons with physical health concerns, significant mental health, addictions and/or cognitive issues such as dementia.
What You Will Do
- Undertakes psychosocial assessments.
- Develops and implements a service plan with clients with significant mental health issues and/or cognitive impairment such as Alzheimer’s/dementia, physical health issues, addictions and/or other geriatric concerns.
- Provides case management services including service co-ordination, short-term and long-term planning, and follow up
- Advocates on behalf of clients and/or their family or caregivers.
- Keeps current about the range of services available in other agencies.
- Links clients and/or their family or caregivers to various community resources, and facilitates appropriate referrals to other community support services.
- Provides crisis intervention, practical assistance, and supportive counselling to clients and/or family or caregivers in homes and community settings.
- Provides service that is responsive, appropriate, and culturally and linguistically sensitive to the individual and caregivers needs.
- Liaises with other psychogeriatric services and consults with other service providers.
- Participates in case conferences and clinical meetings in order to ensure the coordination of services and a continuum of care and leads or participates in Coordinated Care Planning.
- Provides consultation to other agency staff or community partners.
- Maintains a client caseload as assigned by program manager.
- Responsible for collection of statistical data required for the accountability of the program.
- Maintains appropriate records and documentation in a timely manner.
What You Bring to the Team
- Masters of Social Work (M.S.W.) required.
- Current registration with the Ontario College of Social Workers and Social Service Workers.
- Minimum of two (2) years of related experience in mental health, geriatric services and/or social work.
- Demonstrated experience working with older adults and seniors required.

- Demonstrated experience supporting individuals with significant mental health and/or cognitive impairments, physical health issues, addictions, and/or geriatric concerns, especially with at risk, marginalized older adult populations
- Proven ability in case management and supportive counselling.
- Strong assessment and clinical skills.
- Demonstrated experience in application of therapeutic strategies and interventions.
- Effective oral and written communication and interpersonal skills.
- Self-directed and independent, but also proven experience in working effectively as a team member in collaboration with other community professionals.
- Strong time management and organization skills.
- Demonstrated documentation skills.
- Ability to exercise good judgment and flexibility.
- Ability to work from a client-centred approach.
- Knowledge of issues affecting older adults and geriatric populations.
- Comprehensive understanding and knowledge of the health sector and related services.
- Excellent knowledge of community resources.
- A thorough understanding of clinical practices and care models.
- Applied computer skills (MS word, excel, Internet).
- Ability to use case management software (Pirouette) is an asset.
- Experience with OCAN or Inter-Rai CHA assessment tool is an asset.
- Ability to communicate in a second language is an asset.
