Social Worker - Permanent Part-time

You possess at least three years of experience in the provision of evidence-based practice and management of a diverse range of clinical and psychosocial issues in hospitalized patients, particularly the elderly.

Your degree in social work and current registration with the Ontario College of Social Workers and Social Service Workers is complemented with two to three years of hospital experience utilizing evidence-based practices to manage a range of clinical and psychosocial issues in hospitalized patients, particularly the elderly, and well-developed knowledge of hospital and community roles and services including the professional resources available in the care of patients.

You have a broad range of knowledge of inpatient and outpatient services and a demonstrated ability to work successfully in an interdisciplinary environment.
