The Renfrew Victoria Hospital (RVH) is a fully accredited facility located just 45 minutes west of Ottawa. We are currently recruiting for a permanent part time Social Worker/Discharge planner. 

Working as a member of the clinical team, the Social Worker/Discharge planner assists the provider (physician, NP), patient and family with the implementation and planning of the arrangements for the level of care, whether it be in another hospital, a care facility or in the patient’s home.  

In this role, you would also support patient care delivery within the Diabetes program of the Nephrology department by supporting patients and families to understand the plan of care by coordinating care and facilitating choice so that care is individualized according to each patient’s needs.

If you possess a Baccalaureate or Master in Social Work or Baccalaureate with equivalent experience and are a member of the OCSWSSW, or related professional organization and have a passion for this area of Social Worker, this will be a perfect role for you. 

As Social Worker/Discharge Planner, experience with individualized discharge care plans and in the dialysis discipline working with clients with diabetes will be considered strong assets.  As the program is designated under the French Language Services Act, bilingualism will also be considered an asset.
