Lakeview Manor
This position is located at Lakeview Manor in Beaverton, Ontario.

Social Worker
Reporting to the Director of Care, the Social Worker is responsible for providing support, counselling and education to
residents, family members, staff and the community at large to create a harmonious living and working environment, under
the umbrella of Innovative Person-Centered Care.

Specifically, the incumbent will:
- Encourage and support an Innovative Person-Centered Care (IPPC) environment
- Respond to inquiries from all stakeholders about the Home's programs and services
- Coordinate and support internal transfers in conjunction with appropriate legislation
- Provide supportive counselling services and coordinate problem solving meetings with residents, families and other
stakeholders, as deemed necessary
- Complete resident post admission psycho-social assessments to meet the MOHLTC regulations
- Engage with various community-based agencies or programs, as deemed necessary
- Participate and engage in various internal committees and meetings, such as Palliative Care, Ethics, P.I.E.C.E.S. and
Family Support Group
- Develop and deliver education on various regional and home specific policies and procedures, including problem solving,
communication and abuse free care
- Schedule and attend care conferences as required
- Participate in CQI initiatives and Accreditation process
- Work with the interprofessional teams to coordinate resident specific services, such as memorial services and final

- Be available to work weekends and evenings, as needed

The successful applicant will possess:
- A University Degree in Social Sciences (Psychology, Sociology or Social Work) or equivalent; a Master's Degree in
Social Work is preferred
- Current registration with the Ontario College of Social Workers and Social Services Workers as a Social Worker
- Three to five years relevant experience in a health care setting, preferably working in a residential setting such as a longterm
care facility
- Comprehensive knowledge of Ministry of Health and Long-Term Care standards and legislation, and other relevant
policies, procedures and legislation
- Demonstrated problem solving, conflict resolution, mediation and counselling skills
- Excellent written and verbal communication skills, coupled with strong facilitation and presentation abilities
- Working knowledge of MS Office, including Word, Excel and PowerPoint
- Extensive knowledge in palliative care and the use of ethical frameworks when solving problems
- P.I.E.C.E.S certification preferred
- Experience with Electronic health care record (eHCR)
- Proven organizational and time management skills
- The ability to work collaboratively with residents, family members and members of the interdisciplinary health care team

Management & Exempt Salary Grade 4
- Salary: $75,506 to $94,382 per annum

Conditions of Employment
The successful candidate will be required to provide a satisfactory Police Vulnerable Sector Check dated within two (2)
months from date of hire. Additionally, tuberculosis screening will be required in accordance with the LTCH Act (O.Reg.
79/10). Proof of education, qualifications and any other job bona fide requirements will also be collected.
