The Charles H. Best Diabetes Centre is seeking a skilled social worker who is passionate about providing therapeutic services to a patient base varying in age and areas of need; all with an underlying chronic illness, type 1 diabetes. You must be comfortable with a wide scope of mental health issues and find reward in being flexible in your practice. The person in this role will also be responsible for complex case management both directly, by partnering with family members and community agencies, and indirectly, by supporting the clinical interdisciplinary team and administrative support team. You will report directly to the Executive Director.

 

This 12 month 0.6 FTE contract position will support the patients of the Centre; hours and days of the week worked to be negotiated. With a competitive hourly salary, the successful candidate will also receive 11% pay in-lieu of benefits and 6% pay in-lieu of vacation.

 

Background Information

The Charles H. Best Diabetes Centre is a one-of-a-kind community healthcare centre (and a registered charity) that exclusively serves the needs of patients living with type 1 diabetes. We support patients of all ages and work closely with their families and caregivers. We are with our patients throughout all stages of their lives, providing care to children and adults and their caregivers, seeing teens through the transition to adulthood and working with adults through lifecycle changes such as pregnancy and into the senior years.  

We offer intense clinical programming and services; based on a truly integrated and collaborative care model. Our interdisciplinary team includes Registered Nurses, Registered Dietitians, Registered Social Workers, a Registered Practical Nurse, an Administrative team and an Executive Director. We are also affiliated with many physicians, both pediatric and adult, who run clinics at our Centre.

Key Responsibilities

• Formulate and deliver assessment and evidence based treatment plans in partnership with the client, family and interdisciplinary team to achieve progress toward goals ( including management of crisis and life-threatening behaviours).

• Conduct individual, couple, and family therapy sessions across all ages of the lifespan with a special focus on the pediatric population.

• Develop and facilitate group psychoeducation and therapeutic sessions.

• Provide system navigation, resources, case management and advocacy (home, school and community).

• Liaise with community partners or create new alliances as needed to support the care plan.

• Maintain a flexible approach by multi-tasking and aptly re-prioritizing to manage conflicting priorities/demands on your time.

• Ensure quality and integrity of all patient information through use of electronic medical records.

• Maintain close communication with the senior social worker and interdisciplinary team members to ensure continuity and efficiency of the education and support being provided to our patients and families.

• Contribute to the achievement of Centre’s 2019-2024 Strategic Plan as published on our website.

• Work with team members to build and maintain a respectful, healthy and engaged work environment.

 

 

Qualifications

• A Master of Social Work degree 

• Current membership in good standing with the OCSWSSW

• Proven ability to practice with clinical autonomy with a commitment to collaborative practice

• Proof of professional liability insurance

• Clinical experience in the assessment and treatment of children, youth and complex families in a hospital-based or community-based clinical team

• Experience with evidence based treatment modalities, including Cognitive Behavioural Therapy(including DBT), motivational interviewing, mindfulness, crisis intervention, family therapy and group facilitation

• Experience with disordered eating is an asset

• Accurate, thorough and timely documentation skills

• Knowledge of type 1 diabetes is an asset but not required

• Demonstrated commitment to ongoing and self-directed professional education and development
