Position Summary

Who we are looking for?

Ashcroft B.C. has an exciting opportunity for a permanent full-time Social Worker to join their team at the Urgent Primary & Community Care Centre. This position works Monday to Friday from 08:00 to 16:00 with weekends and stat holidays off.

What will you work on?

In this role, you will provide comprehensive assessment, care planning and treatment coordination for designated clients and their families. Working collaboratively with other health care providers and the multidisciplinary team, you will provide direct psychosocial care, counseling, system navigation and advocacy, education and linkage to relevant specialized complex care resources while facilitating client self-management. As the Social Worker you will be responsible for providing and developing social work services to maintain or improve the health and social well-being and functioning of the identified population.

The new UPCC will provide comprehensive primary care services with a team of general practitioners, registered nurses, social workers, physiotherapists and medical office assistants. Qualifications

Education, Training and Experience:
Bachelor’s Degree in Social Work from an accredited university.
Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training
and experience.
Current full registration with the British Columbia College of Social Workers.
Current valid BC driver’s license.
