At The Ottawa Hospital, we understand that a new diagnosis, illness or hospitalization can be a very stressful experience. That’s why social workers are assigned to patient care areas across all The Ottawa Hospital campuses.

Social workers are professionals dedicated to addressing the psychosocial needs of patients and families as part of the health-care team. Social Work uses the social determinants of health (SDOH) as the framework for their practice. SDOH includes such things as social supports, employment, housing, income, gender and culture.

All the over 100 permanently employed social workers at The Ottawa Hospital hold a master’s degree in social work from an accredited University.  All social workers at The Ottawa Hospital are registered with the Ontario College of Social workers and Social Service Workers.

We are seeking social workers to do casual relief work on inpatient units with a discharge planning focus. 

What you’ll do

The casual relief social work positions have a primary focus on discharge planning.  The common day to day tasks includes- completing psychosocial assessments, developing comprehensive discharge plans, facilitating referrals and connection to post acute and community resources and services while provide supportive care to patients and families to ensure safe and timely discharge plans. 

In addition to discharge planning, as part of the interdisciplinary health-care team, the social workers at The Ottawa Hospital provides variety of services:

  • Support and resources in the event of catastrophic or chronic illness
  • Assistance with coping with diagnosis, illness or hospitalization
  • Support for family members
  • Grief and loss counselling
  • Crisis intervention
  • Caregiver support
  • Facilitation of family meetings/conferences
  • Links to programs and services to address financial difficulties
  • Community resource information and referral
  • Support groups

What you’ll bring

  • Master's in Social Work with a clinical concentration from a university of recognized standing; 
  •  Member in good standing of the Ontario College of Social Workers and Social Service Workers;
  •  Experience working in an acute care hospital setting;
  •  Knowledge of a variety of theoretical frameworks and treatment modalities such as crisis intervention, systems theory, etc.
  •  Knowledge of the Health Care Delivery System;
  •  Knowledge of legislation, acts and regulations such as Ontario Works, Public Hospital and Mental Health Acts;
  •  Experience working with issues such as dying, grief and loss;
  •   Ability to provide both group and individual counseling;
  •  Excellent knowledge of discharge planning, community resources and the placement process;
  •  Excellent assessment and crisis intervention skills;
  •  Demonstrated skills in teaching and consultation;
  •  Superior written, verbal and presentation skills;
  •  Ability to multi-task;
  •  Superior interpersonal skills;
  •  Must be eligible to work in Canada.

     

    You’ll stand out with

  •  CAS experience;
  •  Proficiency in English and French – oral expression (advanced level) and comprehension (advanced level).
